HeatherwoodAccess the public car park via the barrier. Take the ticket and as you go into the hospital entrance the Security office is on the left hand side before the reception area. They will validate the ticket (after seeing your Maidenhead Care ID) so that you can exit the car park.

Completed forms should be returned to the treasurer, ideally by email to treasurer@maidenheadcare.org.uk Reimbursement is made by direct bank transfer to your account.

These can be printed off, completed and given to clients which may allow them to reclaim from the NHS the cost of their donation.
